• Insurance information

The following insurance plans are accepted:

• Blue Cross / Blue Shield, Fidelis, Humana, Independent Health, Medicare, Medicaid NY, Molina, Univera, & United HealthCare

We are unable to accept these insurance plans:

• Aetna, Cigna, MVP, The Empire Plan, UPMC, United Healthcare Dual Plan, or Railroad Medicare. The cash price applies to these plans.
